Go to Chunghwa Telecom outlets or business office in person

1. Requirements for Individuals
   • Prepair 2 ID documents：
     • Primary ID : passport
     • Secondary ID：ARC or National Health Insurance IC card or VISA or Driver’s License
   • A chop or signature is needed on the application form.
   • deposit NT 2,900 or
   • be accompanied by a joint guarantor who must be ROC citizen over 20 years old and hold an I.D. Card and NHI IC Card (or Driving License).
   • If you ask a substitute to do it for you , the substitute should carry his own original copy of 2 ID and copy of the registrar’s 2 ID mentioned above.
2. Requirements for Business
   • Business Certificates
   • The company`s chop on the application form.
   • The CEO`s ( or representative`s) I.D. Card and NHI IC Card (or Driver`s License) and chop or signature..
   • If you ask a substitute to do it for you , the substitute should carry his own original copy of 2 ID and copy of the registrar’s the above ID documents.
